Figured out what it was. Whenever you have your aim setting on anything but "Free Aim" this bug can happen. If anyone else experiences this bug, switch your aim setting to this in the Controls menu under Settings in the pause menu.
You have so much extra time to aim for that headshot that this shouldn't be an issue lol.
One other thing I noticed about this mod: every once in awhile in LT-to-aim mode, I'll press it and nothing happens for a second or two, then slow mo engages. Is there a fix for this?
If it helps, I also notices that the slow mo always engages when you hold down the LT part of the way, but not when you slam it down all the way. What is happening here? I looked at the script and I can't figure it out.

As for the Fleeca thing, are you saying you can't even place a ped to spawn behind the counter? The player doesn't really have to get behind the counter, just be able to point a gun at someone behind it.
As for the cash spawning after the hold up, you could just have it spawn near the vault instead of behind the counter like it does in convenience stores.
Or you could just do what you do with the safety deposit boxes at the bank: once you fill the gun-pointing bar, the "looting" condition is met. Players have to go to the drop off to get the real cash anyway.
If you really can't get someone behind the counter, that totally sucks, but once we have more tools to work with all of this will be easier. You could always spawn the ped in the vault area.
Anyway, gonna go download steam.
While I could give you a ton of overscoped suggestions about how to make this mod perfect, I'd rather give you specific suggestions about the next iterative version of the mod to fix some of the game feel.
I really like the bank mission and how full it feels. It's the perfect amount of tension and extreme difficulty for the reward it pays out. Actual planning is needed to pull this off, so it feels like a real free form heist. It may be slightly too difficult for some, but I really enjoyed the challenge here. Four stars is also just perfect because the police will start storming the place as soon as you're about to make your way back up and out, and it's relatively difficult to escape them after leaving, so the chase is epic.
Once I got LUA working, I really enjoyed playing the bank multiple times and taking multiple approaches, so my suggestion here is to perhaps randomize the number of spawning of guards or other events within the bank so that people who complain about difficulty have the luck of the draw when it comes to how many guards happen to be on shift. This also will reward people who want to play it multiple times through.
My main problem with the mod, however, are the Fleeca branches. Just busting in and killing two guards feels too easy and not that tense. Same with the mechanic of just pressing a button to steal all the cash. Part of what makes the bank job work so well is its procedural nature: you have so much to do and almost no time to do it before SWAT rains down from the heavens.
What I suggest is adding a mechanic similar to the way convenience store robberies work: a teller behind the counter spawns in the interior and possibly uses the same script as the convenience store clerk, so you have to keep a gun on them as they pack up your cash. This adds tension to gameplay because you would constantly have to cover the teller while your back is totally exposed to the door behind you, meaning cops have a shot at you.
Adding even a single random ped who simply just cowers in place in the bank line after you open fire on the guards also would go a long way in making this really feel like a robbery. I like that all the peds flee in the big bank though, so don't change that. It coincides with the big firefight that ensues during your escape (you have no hostages, why shouldn't the cops storm the place?).
I noticed also that as soon as you shoot a guard outside of the Fleeca branch, the doors instantly lock. Getting the doors not to lock in this situation might be difficult to figure out, but maybe you could just make sure that all guards spawn only inside the branch? Then you would always have to be shooting in to kill them anyway. I kind of like that the doors can lock if you're not careful too. It feels like something that might really happen in an emergency at a bank. You just don't want players to accidentally kill the guard outside the Fleeca branch their first try of your mod only to think it's broken because the doors won't open.
Also, last thing. I use a controller while playing, could you make this compatible with using the action button on the D-pad too?

I think I'm having the same problem as a lot of other people in that everything works great in the mod (I can enter the interiors fine and initiate the heist by shooting guards), but once I actually try to get money at any one of the locations, no cash is added to my inventory.
Its worse at the bank, it won't give me the bag or start grabbing money at all. The cash on the table never even shows up for me. I have installed both Script Hook and LUA plugin; I mean I've used other mods before, its not like this is my first rodeo.
Really love the idea for this mod. Wish that I could get it to work. Thanks for your content!
Really fun to mess around with. I can tell this is a pretty early proof of concept, but a fun and promising one nonetheless. Releasing a less immersive version might be advisable: a version that doesn't touch time or weather, lets police and peds stay in the game, and allows zombies to agro and attack everyone.

For as well commented as the actual script is, there no readme file, so beginners probably won't know how to install this

I was looking at your script about to attempt to write a few lines in it myself, but it is nice to have the option to be able to have global bullet time, so I think I'll just leave it. The fists trick is the perfect compromise.
